  1. What is SIERA?
    SIERA is the System for Integrated Electronic Reporting and Auditing. SIERA is an HCAI application that allows health facility and pharmaceutical manufacturer employees and consultants to submit and revise HCAI reports, view past reports, maintain user information, and perform other report functions.

  2. What days and hours is SIERA available?
    SIERA is available 24 hours a day, seven days a week, except when routine maintenance needs to be performed.

  3. Can I create a bookmark to get direct access to SIERA?
    Yes. The bookmark should use the web address https://siera.hcai.ca.gov. That address will open the SIERA Portal Page, where you then click the Login button to begin the HCAI multi-factor authentication process to access SIERA.

  4. What internet browsers can I use for SIERA?
    SIERA can be used with Google Chrome, Microsoft Edge, and Mozilla Firefox. Other browsers can be used, but they may not provide complete functionality in SIERA.

  5. My computer screen does not display all of the information on the SIERA page. What can I do?
    SIERA was designed for a screen resolution of 1280x960. If your computer is set to a lower resolution, such as 1024x768, you can still use SIERA. However, you may need to scroll left and right and up and down on some pages to see all of the information.

  6. Some of the buttons on my page overlap and do not function. What can I do?
    This issue is usually caused when your browser window is not set to the recommended size and zoom setting. We recommend that as a best practice set your window to Maximize size and the Zoom setting to 100%.

  7. Is my user information visible to other SIERA users?
    Users who do not work for HCAI are typically limited to seeing only your name and organization. Some report preparer information entered on reports may be visible to the public (except for email addresses). HCAI staff who support SIERA can view all of your user information except for passwords.

  9. How do I register in HCAI’s multi-factor authentication process to access SIERA?
    Employees or consultants for healthcare facilities or pharmaceutical manufacturers must have an HCAI multi-factor authentication (MFA) account in order to access SIERA or other HCAI systems. If you need access to SIERA but do not have an HCAI MFA account, then contact your facility or manufacturer’s SIERA primary contact or user account representative to request access to SIERA. If approved, you will be added to SIERA and receive an email with instructions on how to register and login to SIERA.

  10. I was informed that I should have received an email from SIERA with a temporary password to register into the HCAI multi-factor authentication process, but I did not receive one or no longer have the email. What should I do to access SIERA?
    If you do not have or did not receive an email from SIERA with a temporary password, and you were already a SIERA user, then first check your spam or junk email folders to verify they were not sent there automatically. If the email is not in those folders, then click the Forgot My Password link located in the HCAI Sign In page (the page that opens after you click Login on the SIERA Portal page (https://siera.hcai/ca.gov)). The HCAI multi-factor authentication process will guide you through the steps to verify your email address and create a new password.

    Go to the SIERA Portal page (https://siera.hcai.ca.gov) and click the Login button to begin the HCAI multi-factor authentication process to access SIERA.

  13. I am logging into SIERA. At the HCAI Sign-In page, do I enter my email address and password, or do I click the “HCAI Staff Only” button?
    Users who are employees or consultants for healthcare facilities or pharmaceutical manufacturers must enter their email address and password in the HCAI Sign-In page for access to SIERA. The “HCAI Staff Only” button is only used by HCAI employees.

  14. I tried to login to SIERA but I received a message that said "We can't seem to find your account". What should I do?
    SIERA requires an active HCAI multi-factor authentication account be established after May 1, 2024 in order to access SIERA. If you previously had access to SIERA and are now receiving the message that says "We can't seem to find your account", then contact HCAI for assistance.

  17. I forgot my password. What should I do?
    Click the Forgot My Password link located in the HCAI Sign In page (the page that opens after you click Login on the SIERA Portal page). The HCAI multi-factor authentication process will guide you through the steps to verify your email address and create a new password.

  18. I tried logging into SIERA, but I get to the SIERA Checkpoint page and cannot proceed. What should I do?
    If you are sent to the Checkpoint page then you have been verified by HCAI’s multi-factor authentication (MFA) process, but 1) you have not yet been authorized or added to SIERA as a user, or 2) you are an authorized SIERA user but the link between the HCAI MFA account and SIERA account has not yet been established. For either situation, contact HCAI for assistance.

  20. I received an email from Microsoft with the subject line “Department of Health Care Access and Information B2C account email verification code”. Is this email legitimate, and what is it used for?
    The email you received was sent to you by HCAI’s multi-factor authentication process during login to the SIERA system. The email will come from msonlineservicesteam@microsoftonline.com. It contains a verification code used to authenticate your email address and password. The code expires 5 minutes after receipt.
    If you were in the process of logging into SIERA, then this email is legitimate and provides information you need to access SIERA. If you received this email but were not attempting to login to SIERA, or if the sender email address is not msonlineservicesteam@microsoftonline.com, then please contact HCAI.

  21. I am not receiving HCAI MFA verification code emails when I register or login.
    The verification code emails are sent by Microsoft on behalf of HCAI to the email address we have in our records. First, check your spam or junk email folders to verify they were not sent there automatically. Next, contact HCAI to verify that the email address we have for you is correct. Lastly, contact your facility’s IT support team to determine if the email address that verification emails are sent from is blocked. All verification code emails sent by Microsoft on behalf of HCAI will be sent from the email address msonlineservicesteam@microsoftonline.com.

    To obtain a SIERA user profile and password just ask the SIERA primary contact at your organization to add you via the Add a User page in SIERA. You can also contact the HCAI SIERA support teams:

    Note, if you are the primary contact for a facility or manufacturer, then another user will need to replace you as the primary contact before you can be removed.

  25. I would like to use more than one email address for access to SIERA. What should I do?
    SIERA, and the HCAI multi-factor authentication (MFA) process that controls access to SIERA, only allows one email address per user. If you need to have more than one email address, then you will need to set up more than one HCAI MFA account and one SIERA user profile. You can contact the HCAI SIERA support teams:

    SIERA only allows one person to be designated as the primary contact for a facility or manufacturer at one time. You or your organization will need to determine who is the appropriate person to be your organization's primary contact for HCAI reporting purposes.

  30. I am the primary contact for my organization, but will no longer be in that position. What should I do?
    HCAI requires that every facility or manufacturer that reports information to HCAI have a primary contact. If you will no longer be the primary contact for your organization then please contact HCAI to designate another primary contact.
