  1. What is SIERA?
    SIERA is the System for Integrated Electronic Reporting and Auditing. SIERA is a web-based information system that provides health facility employees, home office employees, and consultants the ability to submit and correct annual and quarterly financial reports, request extensions, view past reports, and perform other financial report functions.

  2. Who uses SIERA?
    SIERA is used by health facility employees, home office employees, consultants, as well as OSHPD management and staff.

  3. What days and hours is SIERA available?
    SIERA is available 24 hours a day, seven days a week, except for times for routine maintenance.

  4. Can I create a bookmark to get direct access to SIERA?
    Absolutely. The bookmark should direct you to the SIERA Logon Page. You cannot access SIERA without first logging on through the SIERA Logon Page.

  5. What internet browsers can I use for SIERA?
    SIERA can be used with Microsoft Internet Explorer, Apple Safari, Google Chrome, and Mozilla Firefox. Other browsers can be used, but functionality may be limited.

  6. My computer screen does not display all of the information on the SIERA page. What can I do?
    SIERA was designed for a screen resolution of 1280x960. If your computer is set to a lower resolution, such as 1024x768, you can still use SIERA. However, you may need to scroll on some pages to see all of the information.

  7. Is my contact information visible to other SIERA users?
    OSHPD staff are able to view all of your contact information. Other, non-OSHPD users are only able to view your name and organization. Note, however, that report preparer contact information on the submitted reports is visible to the public (except for email addresses).

  9. I tried to logon to SIERA but I received a message that said the email address cannot be found. What should I do?
    Before allowing access, SIERA requires the entry of an existing email address. Also, make sure that you are using the correct character to separate first and last names, if applicable. For example, the character might be an underscore instead of a hyphen.

  10. I tried to logon to SIERA but I received a message that said the password is invalid or cannot be found. What should I do?
    Passwords are case sensitive. Make sure the caps lock is turned off and that the proper case is entered for each alphabetic character.

  11. I forgot which email address I am supposed to use for SIERA. What should I do?
    Contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov.

  12. I forgot my password. What should I do?
    Click the Forgot Password? link located below the Password box. The Forgot Password page will open. Enter your complete email address (the email address that is your user ID) into the Email box and click the Send button. An email with a link into SIERA will be sent to the email address.

  14. How do I become a SIERA user?
    To obtain a SIERA user ID and password just ask another SIERA user to add you via the Add a User page in SIERA, or contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov for assistance

  15. I do not want to be a SIERA user. How do I remove my SIERA User ID?
    Contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov. Note, if you are the primary contact for a facility, then another user will need to replace you as the primary contact before you can be removed.

  16. I would like to use more than one email address, mailing address, and/or phone number. What should I do?
    SIERA only allows one email address, mailing address, and phone number per user. If you need to have more than one email address, mailing address, or phone number in SIERA, then you will need to set up more than one user account.

  17. I do not understand how to confirm or change my user information. Whom can I contact for assistance?
    Contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov.

  19. Can I still use IHQRS to submit a quarterly report?
    No. SIERA has replaced IHQRS as the method for submitting hospital quarterly reports to OSHPD. For information on how to submit a quarterly report in SIERA, review these FAQs or Help pages. For additional assistance, contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov.

  20. Can I use my IHQRS user ID and password for SIERA?
    No. The user ID and password used to access IHQRS are different from the user ID and password in SIERA. To obtain a SIERA user ID and password just ask another SIERA user to add you via the Add a User page in SIERA, or contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov for assistance.

  21. How do I submit a quarterly report in SIERA?
    Logon to SIERA and go to the Submit Reports tab. In this page, locate the report you want to submit, and click the report’s View/Submit icon to open the Quarterly Report Period Start and End Dates page.

    In this page, either accept the standard dates or change them to match your report’s reporting period. Click the Enter Data/Submit button to open the Quarterly Submission page. In this page, enter financial and utilization data for your facility. When done, click the Save & Validate button. Any edit messages will appear below the Submission Entry section of the page. Review and resolve as many of the edits as possible by changing the data you have entered.

    Once you are satisfied that all the significant edits have been resolved, click the Continue Submission button to proceed to submit the report. When the Continue Submission button is clicked, the Select Certification Method page will open.

    In this page, select who will be the authorized certifier of the report. The choices are:

    • Select the previous certifier, or
    • Select yourself, or
    • Choose another person.

    After the selection is made, the report will either complete the submission process or be placed on hold until the selected certifier accepts the assignment.

  22. Do I have to complete a quarterly submission in one session?
    No. You may begin a quarterly report submission and save your entries for submission later.

  23. My quarterly report does not span a full three-month period. How do I report a partial quarterly period?
    Quarterly reports that cover less than a full-three month period are considered non-standard. To submit a non-standard quarterly report, you must change the dates in the Quarterly Report Period Start and End Dates page. This page opens after clicking the View/Submit icon in the Submit Reports page.

    After a non-standard quarterly report is submitted, the date change must be approved by OSHPD before the report can complete the submission process.

  24. Can I correct a quarterly report in SIERA after it has been submitted?
    Yes. SIERA allows facility, home office, and consultant users to correct quarterly reports after the reports have been submitted.

    Logon to SIERA and go to the My Reports tab. In this page, locate the quarterly report you want to correct, and click the report’s View/Correct icon to open the Quarterly Correction page.

    In this page, you can change any of the financial and utilization data in the report. When done, click the Save & Validate button. Any edit messages will appear in the Edits section of the page. Review and resolve as many of the edits as possible by changing the data in the report.

    Once you have completed all of your corrections and are satisfied that all the significant edits have been resolved, click the Submit button to notify OSHPD of your corrections. OSHPD staff will review the corrections and either accept or reject them. After acceptance, the quarterly report will be available again for further corrections, if necessary.

  25. The quarterly report I want to submit or correct has a red lock indicator. What does this mean?
    A quarterly report that has a red lock indicator is currently being submitted or corrected by another user. You may view the report but you cannot submit or correct the report until the other user has completed their submission or corrections. You can see who is currently working on the report by hovering your mouse over the red lock icon.

    If you need to submit or correct a report that has a red lock indicator, contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov for assistance.

  26. The quarterly report I want to submit or correct has a blue lock indicator. What does that mean?
    A quarterly report that has a blue lock indicator is currently being submitted or corrected by you. You have sole control over entering or correcting data in the report until you submit the report or corrections.

  27. I began to submit or correct a quarterly report by mistake. What can I do to cancel the submission/correction?
    Go to the Quarterly Submission or the Quarterly Correction page (whichever is applicable) and click the Cancel Submission or Cancel Correction link. All of the entries made since the submission or correction was started will be deleted.

  29. How do I submit an annual report in SIERA?
    Logon to SIERA and go to the Submit Reports tab. In this page, locate the annual report you want to submit, and click the report’s View/Submit icon to open the Annual Submission page.

    In this page, you can upload an annual report file that has been created by an authorized third-party software package. You can also upload a file containing an image of a signed certification form.

    When both are uploaded, click the Continue Submission button to proceed to submit the report to OSHPD.

  30. Do I need to use third-party vendor software to prepare my annual financial reports?
    Yes. You must use approved third-party software to prepare your annual financial reports. You must then use SIERA to upload and submit the annual report file, instead of sending the file by email or on diskette.

  31. Can I correct an annual report in SIERA after it has been submitted?
    Yes. SIERA allows health facility employees and consultants to correct annual reports after the reports have been submitted.

    Logon to SIERA and go to the My Reports tab. In this page, first choose the report type by clicking on the appropriate tab (Hospital Annual Reports or LTC Annual Reports). Next, locate the annual report you want to correct, and click the report’s View/Correct icon to open the Annual Corrections page.

    In this page, you can change any of the financial and utilization data in the report. To access a specific report page, click on the page selector dropdown control at the top of the Correction Entry section.

    To correct any of the data just type over the current value with the new value.

    When done, click the Save icon or the Save & Validate button. All of the totals and subtotals will recalculate, and applicable edit messages will appear in the Edits section of the page. Review and resolve as many of the edits as possible by changing the data in the report.

    Once you have completed all of your corrections, click the Submit button to notify OSHPD of your corrections. OSHPD staff will review the corrections and either accept or reject them. After acceptance, the annual report will be available again for further corrections, if necessary.

  32. The annual report I want to submit or correct has a red lock indicator. What does this mean?
    An annual report that has a red lock indicator is currently being submitted or corrected by another user. You may view the report but you cannot submit or correct the report until the other user has completed their submission or corrections. You can see who is currently working on the report by hovering your mouse over the red lock icon.

    If you need to submit or correct a report that has a red lock indicator, contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov for assistance.

  33. The annual report I want to submit or correct has a blue lock indicator. What does that mean?
    An annual report that has a blue lock indicator is currently being submitted or corrected by you. You have sole control over entering or correcting data in the report until you submit the report or corrections.

  34. I began to correct an annual report by mistake. What can I do to cancel the correction?
    Go to the Annual Correction page and click the Cancel Correction link located towards the bottom of the screen in red font. All of the entries made since the correction was started will be deleted.

  35. Primary Contact Questions Go to Top

  36. I am currently assigned as a "consultant" type of user, but need to change it to another value. SIERA will not let me. What should I do?
    Users who have been assigned as “consultant” cannot change their own type of user setting. If you want to change your type of user to “facility employee" or "home office employee”, please contact the SIERA Help Desk.

  37. OSHPD lists me as the primary contact for a facility. However, I would like another person to be the primary contact instead of me. What do I do?
    Contact the SIERA Help Desk at (916) 326-3899 or siera@oshpd.ca.gov. Note that if you are the primary contact for a facility, another user will need to replace you as the primary contact.

  38. OSHPD lists me as the primary contact on a facility. However, I would like another person to be the primary contact in addition to me. What do I do?
    SIERA only allows one person to be designated as the primary contact for a facility at one time.

  39. I am the primary contact for my facility, but will no longer be in that position. What do I need to do?
    OSHPD requires that every health facility that reports financial information to OSHPD have a primary contact. If you will no longer be the primary contact at your health facility then please contact OSHPD to change the primary contact information.
